Dog Nail Trim Release Form

Consent and Risks

You are authorizing a nail trim, and nail trims carry some risk.

I authorize HolliePaws Grooming and Boutique and Newcastle Pet Supply to trim my dog's nails. Reasonable care will be taken, but I understand nail trims carry inherent risks: dogs may move suddenly, which can nick the nail quick; minor bleeding, discomfort, or sensitivity can happen despite reasonable care; staff may apply styptic powder or basic first aid to stop minor bleeding; and stress or movement-related injuries may occur, though uncommon.

Nail Grinding Upgrade

Grinding is an optional $5 add-on for a smoother finish.

Nail grinding files the nails back gradually for a shorter, smoother result, for an extra $5. If my dog does not tolerate clippers, staff may switch to grinding at the same $5 charge when it is safer or more suitable.

Handling and Disclosure

Staff may safely restrain your dog if needed, and you agree to share any concerns.

I authorize staff to use reasonable handling, including restraint or a muzzle, for safety. Staff may stop the service anytime if my dog is unsafe to handle, and I remain responsible for the full fee. I agree to disclose any behavioral concerns, bite history, medical conditions, or past grooming reactions.

Liability Release

You agree not to hold either business liable for risks that can't be fully eliminated.

I release HolliePaws Grooming and Boutique and Newcastle Pet Supply, and their owners, staff, and contractors, from claims or damages arising from the nail trim or grinding, including accidental nicks, stress or movement injuries, my pet's behavior, damage my pet causes to others, or complications from conditions I did not disclose. This does not apply in cases of gross negligence or willful misconduct.

Agreement and Signature

By signing, I confirm I am the owner or authorized representative of this pet, that I have read and agree to all sections above, and that I will hold Newcastle Pet Supply and HolliePaws Grooming and Boutique harmless from third-party claims arising from my pet's behavior or any injury or damage my pet causes.
